Credit Flexibility Family Guide
Follow the instructions below to complete your Credit Flexibility Applications. Click on the links to view the needed forms.
Step 1
Credit Flex
Intent Form (Form A)
Must be turned in by March 30 for a summer or
fall course.
Must be turned in by November 30 for a spring course.
Step 2
Sample
Credit Flex Proposal
Proposal must be submitted by
May 1 for a summer or fall course.
Proposal must be submitted by December 15 for a spring course.
Credit
Demonstration Packet (Form B)
This form should accompany your proposal.
Fill out page 1 and Administration will complete pages 2 and 3.
Planning
and Tracking Form (Form C)
This form will be used by
Administrator/Facilitator/Evaluator to determine if the student makes progress
toward the goals.
**Forms B and C should be attached to your proposal and used for test out or demonstration options.
Educational
Options Packet - Administration (Form D)
Form D should be attached to
your proposal.
The Administration will fill out this document when your activity is approved or
needs revised.
Progress
Summary Form (Form E)
Form E is a way for you to
plan and track your progress. It will be filled out by you in association
with the Administrator or Facilitator.
**Forms D and E should be attached to your proposal and used for educational options.
